Freckle removal: laser, light and what is realistic
Freckles are an inherited trait and harmless. Many people like them, while others would prefer a more even skin tone. Here you will find out how freckles form, how to tell them apart from age spots and melasma, which technique removes them or merely lightens them, and why they come back without sun protection.
The SIACSKIN solutions you will get to know are the DERMAFORCE BBL PRO for whole areas, the picosecond laser PICOLO XL and the Q-switched lasers PICOLO L and PICOLO S for individual freckles, and the non-ablative TULIX. Also covered: number of sessions, healing and risks.
What are freckles and why do some people get them?
Freckles are small, flat, light brown spots that form when the skin's pigment cells respond to UV light with a particularly large amount of melanin. The tendency is inherited and occurs mainly in people with fair skin and blonde or red hair. The number of pigment cells is not increased; they simply work harder. That is why freckles usually appear as early as childhood, darken in summer and fade in winter. The medical term for them is ephelides.
Because the pigment lies in the epidermis, freckles respond well to light and laser. At SIACSKIN, the options for this are the DERMAFORCE BBL PRO for whole areas and the PICOLO XL with 532 nm for individual freckles.
Freckles or age spots: how can I tell the difference?
Freckles are small, numerous and change with the seasons, whereas age spots are larger, individually defined and stay the same all year round. Freckles usually measure 1 to 3 mm, appear as early as childhood and sit on the nose, cheeks, shoulders and arms. Age spots only develop after many years of sun, usually from midlife onwards, and are often darker. For treatment this means that freckles tend to be treated over a whole area, while age spots are often treated individually.
The Skin Analyzer Pro captures the skin under white light, UV and polarised light and assesses the pigment using AI. This does not replace a medical diagnosis. The DERMAFORCE BBL PRO is suitable for whole areas, the PICOLO L with 532 nm for individual spots.
Freckles or melasma: why does the difference matter before treatment?
The distinction matters because freckles tolerate short, powerful pulses well, whereas melasma often responds to the same treatment with darker patches. Freckles are many small, sharply defined dots that have usually been there since childhood. Melasma appears as larger brownish patches with blurred edges, usually symmetrical on the forehead, cheeks and upper lip, and is linked to hormones, light and heat. If both are present at the same time, treatment is guided by the melasma: low energy, several sessions and strict sun protection.
The Skin Analyzer Pro captures the skin under white light, UV light and polarised light, and under UV light it also reveals pigment in deeper layers. For melasma, SIACSKIN offers the non-ablative TULIX with 1927 nm, and for freckles alone on fair skin the DERMAFORCE BBL PRO.
The most effective are very short-pulsed pigment lasers with 532 nm, that is, picosecond lasers and Q-switched lasers. Green light at 532 nm is strongly absorbed by melanin and penetrates hardly any deeper than the epidermis, which is exactly where the pigment in freckles sits. The short pulse breaks up the pigment without heating the surrounding tissue much. This works well on fair skin. With darker skin types the risk of pigment changes increases, so the dosage is more cautious there.
The PICOLO XL works as a picosecond laser with 280 ps and offers 532 nm with spot sizes of 2 to 10 mm. The PICOLO L and the PICOLO S are Q-switched lasers in the nanosecond range that also provide 532 nm and are intended for superficial pigment disorders.
BBL or laser: which suits lots of small freckles on the face?
For lots of small freckles spread over a whole area, a light treatment usually suits better; for a few pronounced freckles, the laser. With broadband light you treat the nose, cheeks and forehead in a single pass. The complexion as a whole becomes more even, and redness is addressed at the same time. Lasering hundreds of freckles dot by dot would be laborious and can look patchy. The laser shows its strength with individual dark freckles and with whatever remains after the light series.
The DERMAFORCE BBL PRO is not a laser but BroadBand Light with 400 to 800 nm, 9 filters and light windows of 45 × 15 mm and 30 × 12 mm, intended for skin types I to IV. Anything that remains can be touched up with 532 nm on the PICOLO XL.
How many sessions does it take to remove freckles?
With a pigment laser, 1 to 3 sessions are usually enough, while 3 to 5 sessions are typical for a light series. The interval is about 4 weeks for laser and 3 to 4 weeks for light. That is how long the epidermis needs to renew itself and shed the loosened pigment, and only then can the result be assessed. Dark freckles respond faster than pale ones because they absorb more energy. As the predisposition remains, many people plan 1 to 2 top-up sessions a year, ideally in autumn or winter.
With the DERMAFORCE BBL PRO, a session takes 15 to 30 minutes depending on the area. You treat individual freckles with pinpoint accuracy using the PICOLO XL or the PICOLO L.
What happens during laser freckle removal and does the treatment hurt?
The treatment is short and quite tolerable: each pulse feels like a small, hot pinprick or the snap of a rubber band. First the skin is cleansed, the skin type is determined and the starting condition is photographed. Once the protective eyewear is on, a test pulse follows, and then the area is covered pulse by pulse. Freckles that have been hit correctly immediately turn slightly darker or a little grey, and the surrounding skin reddens. Finally the skin is cooled and sun protection is applied. A numbing cream is rarely needed.
On the PICOLO XL, the integrated AI adapts the parameters to the skin type and treatment area, and fine adjustment is done by hand. The cold air of the KYROMASTER adds comfort, cooling before, during and after the pulses.
How does skin look after laser freckle removal and how long does healing take?
The treated freckles first turn darker, form fine crusts and come away on the face after about 5 to 10 days. Immediately after the pulses the skin is red and feels warm, much like after too much sun. In the days that follow, the face looks more speckled than before because the targeted pigment migrates to the surface. Beneath the crusts, pink skin appears at first, which evens out within a few weeks. What matters is gentle cleansing, plenty of moisture, no scratching, no exfoliation and sun protection factor 50 every day.
After a session with the DERMAFORCE BBL PRO, the redness usually subsides within hours. After pinpoint lasering with the PICOLO XL, the spot can be slightly swollen for 1 to 2 days. You will find the individual care steps in the SIACSKIN aftercare guide.
Do freckles come back after laser treatment and how long do results last?
Yes, freckles can come back, often as early as the following summer, if the skin is exposed to the sun unprotected. The treatment removes the existing pigment, but the inherited tendency of the pigment cells remains. How long the skin stays even therefore comes down to sun protection: sun protection factor 50 on the face every day, a hat or shade around midday and no sunbeds. Those who keep this up usually retain the result considerably longer, and the freckles then often return lighter and in smaller numbers. Without protection, the result can be lost after one season.
The DERMAFORCE BBL PRO is suitable for top-up sessions because a session takes only 15 to 30 minutes and does not involve long downtime. In the SIACSKIN skincare range, the Suncream Sensitiv SPF50 is part of daily aftercare.
Can freckles be removed from the shoulders, arms and décolleté?
Yes, freckles can be lightened on the body too, but the skin there needs more cautious settings and heals more slowly than on the face. After sunburn, the shoulders and back often have larger, star-shaped spots, known in the field as sun spots, which do not fade in winter and need more than one session. The skin on the décolleté is thin and does not forgive too much energy. On the body, the darkened freckles usually only come away after 2 to 3 weeks. Because these areas get a lot of light in summer, the treatment belongs in autumn and winter.
For large areas, the DERMAFORCE BBL PRO has a light window of 45 × 15 mm. For lots of fine spots and dull skin, the TULIX with 1927 nm is an alternative; it lists sun damage and pigment disorders as applications.
What are the risks of laser freckle removal on very fair skin?
On very fair skin the risk is low, but temporary redness, swelling, crusts, occasional small blisters or pinpoint bleeding in the skin and, rarely, light or dark patches on the treated areas are possible. Skin types I and II contain little melanin, so the energy mainly hits the freckles. Problems usually arise from energy that is too high, a fresh tan or sun in the weeks afterwards. Scars are very rare. Light patches resolve more slowly than dark ones. A test spot shows how the skin reacts. Spots that look irregular, are growing or are changing should be seen by a dermatologist beforehand.
On the PICOLO XL, the energy can be set from 100 mJ upwards. The DERMAFORCE BBL PRO comes with a skin contact sensor and sapphire contact cooling.
Can freckles just be lightened instead of removed completely?
Yes, anyone who wants to keep their freckles but tone them down chooses a gentle treatment over the whole area with low energy. Instead of targeting and removing each freckle, the epidermis as a whole is renewed. The freckles become lighter and less contrasting, the skin tone looks more even and the face keeps its character. The usual course is 3 to 5 sessions at intervals of about 4 weeks. The goal should be clearly discussed before the first session so that no more pigment is removed than desired.
The TULIX is non-ablative: its 1927 nm thulium wavelength acts in the epidermis and creates fine microzones through which pigment is shed as the skin renews itself. Downtime is short, and the skin flakes finely for a few days.
Do creams, lemon juice or peels help with freckles?
Creams can lighten freckles slightly over months, while home remedies such as lemon juice do little and irritate the skin. Active ingredients such as vitamin C, niacinamide, azelaic acid or retinoids slow pigment formation and speed up skin renewal. This softens the contrast but does not remove the freckles. Fruit acid peels loosen the outermost horny layer and likewise lighten only slightly. Lemon juice can also leave marks of its own. The greatest effect without a device comes from consistent sun protection with sun protection factor 50, because less new pigment forms without the UV stimulus.
Those who want to achieve more combine skincare with a light treatment. The DERMAFORCE BBL PRO is designed for hyperpigmentation and sun damage. For accompanying care, SIACSKIN offers the Serum Vitamin C and the Suncream Sensitiv SPF50.
Can tattooed freckles (freckle tattoo) be removed again?
Yes, freckles that have been tattooed or applied with permanent make-up can usually be removed or lightened considerably with a pigment laser, but they behave like a tattoo and not like real freckles. The brown inks often contain iron oxide, which can instantly turn dark or grey under the laser pulse. The treatment therefore begins with a test spot in an inconspicuous place, which is observed for a few weeks. Because only a little ink lies shallow in the skin, 2 to 4 sessions at intervals of 6 to 8 weeks are usually enough. A light treatment is not suitable for this.
The PICOLO XL breaks down ink pigments with 280 ps and offers 532 nm and 1064 nm, among others. The PICOLO L works as a Q-switched laser with 1064 nm and 532 nm.
Which device does a studio need for treating freckles?
For most clients, a light treatment for whole areas and a short-pulsed pigment laser for individual freckles are sufficient. Freckles almost always affect fair skin types and whole areas, so broadband light does most of the work and addresses redness and sun damage at the same time. The pigment laser complements it for dark individual freckles and anything that remains, and opens up further services such as tattoo removal and Carbon Peeling. Who is permitted to carry out which treatment is regulated differently from country to country.
At SIACSKIN, these are the DERMAFORCE BBL PRO for skin types I to IV and the PICOLO L as a Q-switched laser or the PICOLO XL as a picosecond laser. You can clarify which combination suits your business in the free consultation.
