What is RF microneedling and how does it work?
RF microneedling combines fine needles with radiofrequency energy: the needles penetrate the skin and only release the heat at depth. The high-frequency current heats the dermis locally at the needle tips to about 60 to 70 °C. There collagen fibres contract, and in the weeks that follow the skin forms new collagen and elastin. With insulated needles only the tip conducts, while the shaft is coated. In this way the skin surface is largely spared.
SIACSKIN offers two systems for this: the EUPHEUS M8 works in bipolar mode at 4 MHz with a needle depth of up to 7 mm, adjustable in 0.1 mm increments. The EUPHEUS V additionally draws the skin in by vacuum and uses gold-plated, insulated needles.
What is the difference between RF microneedling and conventional microneedling?
Conventional microneedling creates only mechanical puncture channels, whereas RF microneedling additionally delivers heat deep into the skin. When needling with a pen or roller, the skin responds solely to the fine injuries. This refreshes the complexion and softens fine lines, but hardly tightens the skin. In RF microneedling, a small heat zone forms at each needle tip, which contracts tissue and triggers considerably stronger collagen remodelling. That is why it is used for lax skin, deeper scars and stretch marks. The heat also seals the puncture channels, so there is less bleeding.
The EUPHEUS M8 regulates RF power from 10 to 400 W and needle depth up to 7 mm. On request it also works without needles, as a pure RF treatment.
RF microneedling or fractional laser: which is better for skin tightening?
For tightening at depth, RF microneedling is usually the better choice, and for renewing the skin surface the fractional laser. Laser light enters via the surface and has its strongest effect there: on crêpey lines, coarse texture and irregularities. The RF needles bypass the surface and deposit the heat directly where firm connective tissue forms. This means shorter downtime, fewer restrictions in summer and more leeway with dark skin. Many treatment plans use both in succession.

RF microneedling or HIFU: which tightens the skin better?
Both tighten, but in different layers: HIFU acts deep beneath the skin, while RF microneedling densifies the skin itself. HIFU focuses ultrasound through the uninjured skin onto points at a depth of several millimetres, on the face down to the connective tissue layer above the muscles. This lifts contours but does not change pores, scars or skin texture. RF microneedling improves firmness and the appearance of the skin at the same time and in return brings 1 to 3 days of redness. Sagging contours speak more in favour of HIFU, thin, crêpey or scarred skin in favour of RF.

What is the right needle depth for RF microneedling?
On the face, 0.5 to 2.5 mm is generally used, on the body 2 to 4 mm and more. The depth determines the layer in which the heat arrives. Thin skin on the forehead and around the eyes tolerates only shallow settings, while cheeks with scars or laxity need 1.5 to 2.5 mm. The thicker skin on the abdomen and thighs requires more depth, as do the sweat glands of the underarm. Often an area is treated in two passes at different depths so that the heat acts in a staggered way.
The EUPHEUS M8 can be set in 0.1 mm increments up to 7 mm and thus covers the face, body and underarms. The EUPHEUS V also regulates needle depth in 0.1 mm increments.
Does RF microneedling help with acne scars and large pores?
Yes, depressed acne scars and enlarged pores are among the most common reasons for RF microneedling, and both respond well. The heat points beneath the scar loosen hardened tissue and stimulate collagen, which raises the base of the scar. Pores look smaller because the tissue around them firms up. Rolling and boxcar scars respond best, very narrow, deep scars less so. One advantage over the laser: the treatment is also readily feasible on tanned or dark skin, and the redness lasts only briefly.

Does RF microneedling tighten wrinkles, the jawline and the neck?
Yes, RF microneedling can smooth wrinkles and firm the jawline and neck, provided the laxity is mild to moderate. The heat contracts the connective tissue, and new collagen and elastin make the skin denser over the following 2 to 3 months. Fine lines, crêpey skin on the cheeks and neck and a jawline that has lost its definition respond well. Pronounced jowls with a lot of excess skin remain a case for surgery, and expression lines for other methods.
Tightening, lifting and wrinkles are applications of the EUPHEUS M8, and its Cold Hammer cools the treated skin. On the neck, where the skin is soft and mobile, the vacuum of the EUPHEUS V helps: it holds the skin in place so that the needles penetrate evenly.
Does RF microneedling help with excessive underarm sweating?
Yes, RF microneedling can considerably reduce excessive underarm sweating because the heat hits the sweat glands themselves. The glands lie at the junction between the dermis and the subcutaneous fat, about 2 to 4 millimetres deep. The needles deposit the radiofrequency precisely there and destroy some of the glands. The usual course is 2 to 3 sessions at intervals of a few weeks. The effect generally lasts considerably longer than with injections, which are repeated every few months, but it can diminish over time. Local anaesthesia is used, and afterwards the underarm is tender to pressure for a few days.
The EUPHEUS M8 explicitly names hyperhidrosis of the underarms as an application and reaches the necessary layer with a needle depth of up to 7 mm.
Yes, RF microneedling is regarded as one of the safest energy-based procedures for dark skin types IV to VI. Radiofrequency heats tissue via its electrical resistance and not via pigment, so melanin plays no part in the effect. In addition, insulated needles only release the energy below the epidermis, in other words beneath the layer in which the pigment-producing cells sit. A residual risk of dark spots remains if treatment is too shallow or uses too much energy. Cautious settings, a test patch and sun protection keep it small.
The EUPHEUS V works with gold-plated, insulated needles. On the EUPHEUS M8 the depth can be set in 0.1 mm increments, so that the heat lands precisely beneath the surface.
What are vacuum and cooling for in RF microneedling?
The vacuum ensures uniform needle depth, and the cooling provides more comfort and calms the skin more quickly. The negative pressure draws the skin into the cartridge and holds it firmly. As a result all the needles penetrate to the same depth, even in soft areas such as the neck, abdomen or upper arms, and the puncture is felt less. Cooling takes the heat from the surface and dampens redness and swelling.

Does RF microneedling hurt and do you need an anaesthetic?
With numbing cream, RF microneedling is easy to tolerate, while entirely without it the treatment is clearly noticeable. The cream is left to take effect for 30 to 45 minutes. What you then feel is pressure and a brief sensation of heat with each pulse. It is stronger over bone such as the forehead and jawline and on the upper lip, and weaker on the cheeks. The deeper the needles and the higher the energy, the more intense it becomes, which is why stronger anaesthesia is often used on the body and the underarms. Afterwards the skin feels warm for a few hours, as after sunburn.

The downtime after RF microneedling is usually 1 to 3 days. Immediately afterwards the skin is red and slightly swollen, which subsides in 24 to 48 hours. A fine dot pattern or tiny crusts may remain visible for a few days, and occasionally there are small bruises. Make-up is usually possible the next day. Exercise, sauna and swimming pool are paused for 2 to 3 days, retinol and acids for about a week, and sun protection is mandatory. After deep treatments on the body, for example for stretch marks, the swelling lasts somewhat longer.

How many RF microneedling sessions are needed and at what intervals?
The usual course is 3 to 4 sessions at intervals of 4 to 6 weeks. For acne scars and stretch marks it is more like 4 to 6. There is a reason for the interval: the skin needs weeks to form new collagen, and an earlier session brings no additional benefit. The first changes appear after 2 to 4 weeks, the full result about three months after the last appointment. A facial treatment takes around 30 to 45 minutes, not counting the time for the anaesthetic to take effect.
With the EUPHEUS M8 and the EUPHEUS V, needle depth can be adjusted in 0.1 mm increments from session to session, for example if the skin reacted strongly or hardly at all to the first appointment.
The tightening generally lasts 1 to 2 years, and the improvement in scars usually remains. The newly formed collagen is the body's own tissue and does not simply disappear again. Skin ageing continues, however, which is why firmness gradually declines. How quickly depends on age, sun, smoking and weight fluctuations. With a top-up session every 6 to 12 months the result can be maintained well.

Are the needle cartridges in RF microneedling consumables and which ones are available?
Yes, needle cartridges are consumables: a fresh, sterile cartridge is used for each client and disposed of afterwards. This is a matter of hygiene, and the fine needles also become blunt. The cartridges differ in the number of needles. A few pins fit small areas such as the nose or the eye area, while many pins work on cheeks and body areas more quickly. Gold-plated needles conduct well and are kind to the skin. The cost per cartridge belongs in the calculation of the treatment price.
