Treating a double chin without surgery: ultrasound, radiofrequency and what is realistic

A double chin rarely has just one cause: usually a fat pad under the chin, declining skin firmness and a person's own anatomy come together. Which non-surgical treatment helps therefore depends on which of these predominates in your case. What causes a double chin: fat, lax skin or genetics?

A double chin usually results from three things at once: a pad of fat beneath the chin, declining skin firmness and the inherited shape of the jaw and neck. Fat beneath the chin is strongly hereditary and is found even in slim people. Over the years the skin loses collagen and elastin, the flat neck muscle slackens and the tissue sinks downwards. A short or receding chin additionally makes the angle between chin and neck look flatter. Weight gain accentuates all of this. What matters for treatment is which factor predominates. How can I tell whether my double chin is made up of fat or lax skin?

Use the pinch test: if you can grasp a thick, soft fold beneath the chin, fat predominates; if the fold is thin and crinkly, lax skin predominates. Also look at yourself side-on in the mirror. If the area under the chin still bulges when your head is raised, volume is behind it. If it almost disappears when you gently push the skin at the angle of the jaw towards the ear, what is mainly lacking is firmness. How does focused ultrasound (HIFU) work on a double chin: does it only tighten or does it also break down fat?

On a double chin, focused ultrasound primarily tightens; it can additionally reduce a small fat pad. The sound waves converge at a fixed depth and heat tiny points there. At depths of 3.0 and 4.5 mm, connective tissue and collagen fibres contract, which lifts the contour beneath the chin. If the focus lies deeper in the subcutaneous fat, fat cells are damaged by the heat and broken down over a period of weeks. This only works if the fat pad is thicker than the selected focal depth. The course of RF microneedling follows 4 weeks later at the earliest; it densifies the skin itself and further sharpens the jawline. Can you exercise away a double chin, or does only losing weight help?

A double chin cannot be exercised away, and losing weight only helps if excess weight is the main cause. Chin exercises strengthen the muscles, but fat cannot be reduced selectively in one area, and lax skin does not become firmer through exercises. Those who lose weight often lose volume beneath the chin too. What sometimes remains is lax skin that continues to spoil the profile. An upright head posture makes the area under the chin look smaller immediately but does not change the tissue. Massages and creams have at most a brief draining effect. What does double chin treatment with ultrasound involve and how long does it take?

Treatment of the area under the chin usually takes 20 to 30 minutes and is carried out without an anaesthetic injection and generally without downtime.
