Treating wrinkles and skin ageing without surgery: methods compared

Wrinkles do not all form in the same way: fine lines, deep wrinkles, sagging contours and dull skin with enlarged pores have different causes and need different methods. This page helps you decide which non-surgical wrinkle treatment suits which concern and which area, from crow's feet and upper lip lines to the neck, décolleté and hands.

You will find out how skin ageing proceeds, what laser, radiofrequency and light can achieve and where their limits lie. The matching SIACSKIN devices are the FRACO CO2 XL and the TULIX for the skin surface, the EUPHEUS M8 and the EUPHEUS V for tightening, and the DERMAFORCE BBL PRO for skin tone and sun damage.

How do wrinkles form and what happens in the skin as it ages?

Wrinkles form because the skin loses collagen and elastin over the years and so becomes thinner, drier and less elastic. From around the mid-20s, the body produces roughly one per cent less collagen each year. Elastic fibres tire, cell renewal slows, and the fat pads and bones of the face diminish. Then there is the sun: UV radiation causes the greater part of visible skin ageing because it breaks down collagen and triggers pigment spots. This is where energy-based treatments come in. They generate controlled heat and prompt the skin to form new collagen. As a fractional CO2 laser, the FRACO CO2 XL renews the skin surface, the EUPHEUS M8 tightens at depth by RF microneedling, and the DERMAFORCE BBL PRO treats sun damage and uneven skin tone.

Fine lines, deep wrinkles or lax skin: which treatment suits what?

Fine lines are treated at the skin surface, deep wrinkles in the dermis and lax skin one level deeper still. Crepey lines and coarse texture sit in the upper layers of the skin and respond to fractional lasers. Deeply etched wrinkles need vigorous collagen remodelling, that is, an ablative laser or RF microneedling. With laxity, firmness is lacking in the connective tissue; here heat at depth is effective. Often all three forms are present at once, in which case treatments are combined. At SIACSKIN, the TULIX covers fine lines non-ablatively and with little downtime. The FRACO CO2 XL is the ablative CO2 laser for pronounced wrinkles. The EUPHEUS M8 delivers radiofrequency via needles to a depth of up to 7 mm and is intended for tightening and lifting.

What helps with crow's feet and fine lines under the eyes without surgery?

Fractional lasers and RF microneedling set to a shallow depth help with crow's feet and crepey lines under the eyes, because both densify the thin skin there. The skin around the eyes is the thinnest on the face, which is why it is the first to show collagen loss and sun damage. Fine lines that are visible even when the face is relaxed can be smoothed in this way. The component that only appears on laughing comes from the circular muscle around the eye and remains. The TULIX works non-ablatively with spots of 50 to 200 µm and is the gentle entry-level option for this area. For more deeply etched lines, the FRACO CO2 XL is the more powerful choice; its heat delivery can be finely dosed via pulse widths from 0.1 ms. On the EUPHEUS M8, the needle depth can be set in 0.1 mm increments, which is necessary with such thin skin.

Can a laser smooth forehead lines and the frown line, or is botulinum toxin needed?

A laser smooths the lines that have already become etched into the skin; it does not stop the muscle movement behind them. Forehead lines and the frown line are expression lines: they form because the skin over active muscles is repeatedly pushed together. As long as the muscle is working, the line re-forms. Botulinum toxin is the appropriate remedy for that. Laser and radiofrequency improve what remains visible at rest: the etched line, thin skin, coarse texture. The two complement each other well. The FRACO CO2 XL treats such etched lines ablatively. The TULIX is the gentler, non-ablative variant for fine horizontal lines. If the forehead skin as a whole is to become firmer, RF microneedling with the EUPHEUS M8 is added.

How can nasolabial folds be softened without surgery and without fillers?

Nasolabial folds can be softened without fillers by tightening and densifying the skin of the cheek, though this does not make them disappear completely. The fold between the nose and the corner of the mouth is not a skin wrinkle in the strict sense. It deepens because the fat pad of the cheek descends and volume is lost. Heat in the dermis firms the tissue, the fold looks shallower and the transition softer. With mild to moderate folds this is often enough. If volume is clearly lacking, only volume replacement or a surgical lift compensates for it. The EUPHEUS M8 is intended for tightening, lifting and wrinkles and delivers the radiofrequency to depth via gold-plated needles. The EUPHEUS V additionally draws in the skin by vacuum and also offers needle-free RF tightening.

What helps with upper lip lines and smoker's lines around the mouth?

Fractional laser treatment works best against vertical upper lip lines because the lines sit directly in the skin surface. They are caused by the circular muscle around the mouth, thinning skin, sun and smoking. Creams barely reach them, and fillers can be used only to a limited extent in such fine lines. An ablative laser removes microscopically fine columns, and the skin grows back smoother and denser. Deep lines usually require 2 to 3 sessions. The FRACO CO2 XL is the ablative fractional CO2 laser from SIACSKIN for wrinkles and resurfacing. In CoolPeel mode, according to SIACSKIN, the skin heals in 3 to 4 days instead of 10 to 14, provided appropriate care is used. For incipient fine lines, the non-ablative TULIX is often sufficient.

What helps with neck wrinkles and a crepey décolleté?

On the neck, tightening at depth helps most; on the décolleté, treatment of sun damage and crepey lines at the surface. Both areas have thin skin with few sebaceous glands. They heal more slowly than the face and tolerate only conservative settings. On the neck, horizontal lines and declining firmness are the main concern. After years in the sun, the décolleté shows pigment spots, redness and vertical lines. RF microneedling with the EUPHEUS M8 firms the neck skin, and the needle depth can be adapted to the thin skin in 0.1 mm increments. The TULIX suits the décolleté: it works non-ablatively and is intended for sun damage, fine lines and wrinkles. The DERMAFORCE BBL PRO evens out redness and pigment spots in skin types I to IV.

Can ageing hands be rejuvenated with laser or light?

Yes, on the hands, age spots, thin skin and fine crepey lines can be treated well with light and laser. The back of the hand is exposed to the sun for a lifetime and is seldom protected, which is why it often gives away age earlier than the face. Brown spots respond to light: they initially darken and then flake off within 1 to 3 weeks. A fractional laser improves skin texture over several sessions. Prominent veins and tendons are a consequence of volume loss and remain unaffected. The DERMAFORCE BBL PRO treats hyperpigmentation and sun damage in skin types I to IV. The TULIX is intended for age spots and fine lines and is suitable for all skin types. Afterwards, sunscreen should be applied to the back of the hand every day.

What helps with jowls and a double chin without surgery, and when does ultrasound make sense?

For mild jowls with soft, thin skin, radiofrequency helps; for sagging contours, focused ultrasound is the more suitable method. Jowls form when the tissue of the cheek slides downwards and interrupts the jawline. RF microneedling densifies the skin itself and firms the edge of the jaw. Ultrasound acts deeper, at the connective tissue layer above the musculature, and lifts the contour. A small double chin can be treated at the same time. Where there is marked excess skin, a surgical lift is the only option. The EUPHEUS M8 is intended for tightening and lifting. For the ultrasound lift there is the DERMALIFT, a HIFU device whose applications include non-surgical facelift and double chin. The DERMALIFT PRO works with microfocused ultrasound and likewise lists the double chin as an application.

Why do pores get larger with age and why does the skin look dull?

Pores look larger with age because the collagen framework around them gives way, and the skin becomes dull because it renews itself more slowly. A young pore is round and tautly supported. When firmness declines, it elongates and becomes more noticeable. At the same time, dead keratinised cells remain in place for longer, the surface reflects light unevenly, and small pigment spots appear as well. Both are early signs of skin ageing, often even before the first wrinkles. The TULIX renews the epidermis at 1927 nm and reaches the dermis at 1550 nm; pores and fine lines are among its applications. The DERMAFORCE BBL PRO evens out skin tone with light. For greatly enlarged pores, the FRACO CO2 XL is the more intensive option.

At what age is anti-ageing worthwhile: prevention at 30 or correction later?

Both make sense, but with different means: at 30 it is about preservation, from the mid-40s about correction. In your thirties the collagen framework is still dense. Early lines, pores and pigment spots can be kept well in check with gentle treatments 1 to 2 times a year. The most important element remains daily sun protection. Later, wrinkles are etched in and the skin slackens, and then more intensive procedures and several sessions are needed. Those who start early need less later on, but a great deal can still be made up for. For prevention, the TULIX with its little downtime and the DERMAFORCE BBL PRO for early sun damage are suitable. For correction there are the FRACO CO2 XL and RF microneedling with the EUPHEUS M8.

How do you sensibly combine anti-ageing treatments in an annual plan?

A good annual plan places intensive laser treatments in the months with little sun and tightening and maintenance in the rest of the year. Autumn and winter are the time for ablative sessions, because freshly treated skin does not tolerate sun. In spring a course of RF microneedling follows, usually 3 sessions at intervals of 4 to 6 weeks. In summer the focus is on skincare and sun protection. There should be at least 4 weeks between two procedures on the same area. With SIACSKIN devices it looks like this: in winter the FRACO CO2 XL for the surface, then the TULIX, which is expressly intended as a complement to the CO2 laser, and in spring the EUPHEUS M8 for tightening. In this way surface and depth are treated across the year without overtaxing the skin.

How long does non-surgical wrinkle treatment last and what does maintenance treatment mean?

Depending on the procedure, the result lasts about 1 to 5 years; with one top-up per year it can usually be maintained. Ablative resurfacing has the longest effect, often several years. Non-ablative lasers and RF microneedling build up collagen over a course of treatment, and the effect usually lasts 1 to 2 years. The new collagen is the body's own tissue and remains, but skin ageing continues. Sun protection, not smoking and consistent skincare prolong every result. In practice, maintenance means: after the course of treatment with the FRACO CO2 XL or the EUPHEUS M8, one fixed appointment per year. In between, a gentle session with the TULIX maintains skin quality; it works non-ablatively and with little downtime.

Do men need a different wrinkle treatment from women?

The methods are the same, but men's skin calls for different settings and different planning. On average it is about a fifth thicker, produces more sebum and has a denser collagen framework. Wrinkles come later but then etch themselves in more deeply, especially on the forehead and around the eyes. Thicker skin often needs more energy or depth. Caution with light is called for in the beard area, because it can also reduce beard hairs. Many men also want treatments that are not noticeable on them the next day. The EUPHEUS M8 can be set from 10 to 400 W and to a needle depth of up to 7 mm, and so adapts to thick skin. The TULIX is the option with little downtime. The DERMAFORCE BBL PRO also has filters for hair reduction, so the beard area is left out.

Which devices does a studio or practice need for a complete anti-ageing offering?

A complete anti-ageing offering needs three building blocks: one device for skin tone and sun damage, one for the skin surface and one for tightening. These cover the most common concerns, from pigment spots and redness through fine lines and pores to lax skin. It makes sense to start with the device that suits your own clientele and is booked often. Who may carry out which treatment is regulated differently from country to country. At SIACSKIN these are the DERMAFORCE BBL PRO for light, the TULIX as a non-ablative laser without consumables and the EUPHEUS M8 for RF microneedling. For ablative resurfacing, the FRACO CO2 XL is added. The free consultation will clarify which combination suits your business.
